Even in those 'policed' events (which I don't think it is likely any of us will ever play in, since they seem to only be elite GM) it still seems quite possible to slip things past the arbiters - there was a case not so long ago where Magnus carlsen was shown on camera touching a piece to move it but then changing his mind and moving something else while his opponent was away from the board and never got called on it.
Lose one queen and it is a disaster, Lose 1000 queens and it is just a statistic.

That is an outrageous calumny. It was quite obvious that he was adjusting the piece and hadn't gone through the formality of saying so as his opponent was away from the board.
